Welcome to ACORN's giving opportunities. We are delighted to provide information and resources to assist our donors in making a difference!

Our mission is to develop the financial resources to support, expand and enhance the efforts of ACORN to serve our patients by providing quality medical and dental care.

Over the years it is through the combined efforts of many that ACORN has developed into the organization that it is today providing rural healthcare to North Central Florida neighbors.

Ways to Give

  • Give by Phone - We will gladly accept gifts by phone. To make a credit card contribution, please call (352) 485-1133 ext. 18 between the hours of 8:30 a.m. and 5:00 p.m. Eastern Standard Time, Monday through Friday. You may also leave a message after hours and we'll promptly return your call.
  • Give Online - ACORN is pleased to accept gifts online. To make an online gift through our secure server payment system, click here to visit the Online Donation Page.
  • Give By Mail - If you prefer, you may make a gift and send it to ACORN. Please send a gift of a check or money order by mail to:

  • Estate Planning - Thank you for your interest in including ACORN in your estate planning.

    The specific language which you need to use in your Will is as follows:

    "I give, bequeath and devise to Alachua County Organization for Rural Needs, Inc. (ACORN), a nonprofit corporation, organized under the laws of Florida and having its principal office at 23320 N SR 235, Brooker, FL 32622, [the sum of $________] or [the property described below] or [_______ percent of my residuary estate] for its general uses and purposes."

    This bequest to ACORN is deductible for estate tax purposes. However, before making any change to your Will or beneficiary designation, you should consult with your legal and/or tax advisor.

  • Gifts of Stock - Thank you for your interest in and generous support for ACORN! A gift of stock is a great way to help ACORN reach out to new patients and maintaining our service to existing patients.

    Electronic delivery of stock shares is the most secure and expedient delivery process available and it provides efficient internal control as well as cost savings. However, you may also transfer certificates directly.

    To help you facilitate a gift of stock, please use the following instructions.

    Initiate the Transfer - There are two most common methods of gifting securities to ACORN. The first is Electronic Transfer, through which your Broker can transfer shares using the ACORN DTC number. The other is by physically mailing the paper certificates to ACORN. Either method is welcome, but it is necessary for you to contact your Broker to initiate the transfer process.

    Notification of Donor Intent - Regardless of the method chosen to deliver a gift of stock, the donor or the transferring broker must provide the following information, for audit and acknowledgement purposes:

    1. Donor's name and complete address
    2. Name and number of securities transferred

    Securities Delivered Electronically - The following information will enable your Broker to facilitate an electronic transfer of stock:
